Active Lifestyles Programme

The Active Lifestyles Programme aims to provide the whole community with the opportunity to live a more active and healthy lifestyle. Learning more about physical activity, healthy eating, diet and nutrition has never been easier.

We have a number of options depending on your health, level of fitness and personal interests.  All you have to do is ask your GP or Health Professional to refer you onto the Active Lifestyles Programme.

There are 3 levels of support available.  After being referred by your GP, the Active Lifestyles Team will choose the most appropriate level for you…

Level 1: Early Intervention Programme

The Early Intervention Programme is a physical activity scheme aimed at people aged 16 and above with low-level health conditions.  The idea is that by offering support to those in the early stages of developing poor health, it will stop the development of increasingly poor health in the future.

The Early Intervention Programme includes a 14-week individualised physical activity programme, under the guidance of a GP referral qualified Fitness Instructor, at your most convenient Active Sefton Leisure Facility.

Activities range from gym-based sessions, swimming, classes, walking and cycling, through to community weight management programmes, all tailored to help you reach your desired goals.  The programme also includes full health screening before, during and after, including blood pressure, weight and waist circumference checks. All activities are at a heavily reduced rate, with offer of reduced priced memberships and a further 12-months support for those who successfully complete the programme.

Level 2: Weight Management / Exercise Referral Programme

This level of the programme is for people who suffer with more severe health problems, such as high blood pressure, diabetes, angina, obesity and any other heart disease risk factors.

The Programme has 2 components:

Part a) 12-week Weight Management Programme:

This is a free group programme for people who are overweight or obese and who would benefit from weight management support.   It also allows one to one support for those needing further advice or uncomfortable in a group environment.  The programme increases your knowledge and will give you the skills necessary to achieve and maintain a healthy weight.  It not only covers nutritional advice, but also focuses on goal setting, behaviour change, habits, coping mechanisms and relapses, all of which are essential for weight management.

The Weight Management Programme is available at a number of venues right across the borough, and also includes monthly follow up support for up to 12-months after completion.

All participants taking part in the Weight Management Programme are offered free fruit and vegetable vouchers to help them on their way to five-a-day.

As weight management is most successful through a combination of diet and exercise, all weight management participants will also take part in the Exercise Referral Programme outlined below:

Part (b) 14- week Exercise Referral Programme (age 8+, including family referral):

The Exercise Referral Programme is for both families and individual referrals from the age of 8 and over.

Similar to the Early Intervention Programme, this gives you a 14-week supported programme of physical activity, at a subsidised cost, under the guidance of an Active Lifestyles Referral Officer.  The physical activity programmes on offer include gym usage, classes, swimming and yoga, through to walking, cycling, and community based activities.

The programme includes a full health assessment before, during and after, motivational support throughout, the offer of reduced price membership after the 14-weeks, plus monthly follow up support for 12-months to ensure you continue your healthy lifestyle.

All physical activity programmes are geared towards your individually desired goals, and also gives you access to specific “Active Sefton” sessions to encourage group support and help you to meet new friends.

Level 3: Advanced Dietetic Support

This programme gives one to one weight management support through an Advanced Dietician, for people needing more specialised advice.  It is completely free and includes as many follow up sessions as you may need. After completing the programme, participants can then access the group weight management sessions whilst still having the option to access the exercise referral programme to ensure physical activity is built into lifestyle changes.

All 57 surgeries within Sefton have endorsed and refer to the Active Lifestyle Programme, as well as patients being referred from Aintree, Broadgreen, Southport and Walton Hospitals.
